In order to save our patients time and spare them inconvenience prior to an appointment, we recommend filling out and submitting patient information forms electronically at least 3 days before a scheduled visit.
If you are unable to submit your forms online please arrive fifteen minutes early on the date of your first appointment and be prepared with your insurance card, driver’s license and any recent test results in order to make your visit both faster and more helpful.
Insurance Information
DVU accepts Medicare and most major medical insurance plans. Of course patients are responsible for co-payments and any costs not covered by insurance at the time of their appointment. Patients whose insurance requires a referral must bring the referral with them on the date of their first appointment.
